UPB - Access to Electronic Form W-2/1042-S/1095-C

This document provides instructions for paperless access to Form W-2/1042-S/1095-C for active, terminated, or retired employees.

Access Paperless Form W-2/1042-S/1095-C for Active Employees

To access tax forms, employees should follow these steps:

  1. Log in to the Yearli Portal.
    1. Go to the My UI Info website.
    2. Under the My Pay section, select W-2/1042-S Tax Statement.
    3. Click the green Access Tax Forms button.
    4. Click the Continue button.
    5. Log in using your NetID and Password (you will be prompted to use 2-Factor Authentication).
  2. Select the form you want to retrieve.
    NOTE: If you have not enrolled in paperless delivery for the form you selected, you will receive a Form Not Available message.
  3. Your Form W-2/1042-S/1095-C will display in your browser.

Active employees may access forms from the past seven years by selecting the appropriate form.

Whenever possible, do not use a public or shared computer to access your tax documents. Please be aware that when viewing, saving or printing a tax document from the Yearli Portal, the computer you are using may store a temporary copy.
Delete this file if you are using a public or shared computer!

Access Paperless Form W-2/1042-S/1095-C for Terminated or Retired Employees

If you have enrolled to receive paperless W-2 or 1042-S, and/or 1095-C and are no longer an active employee, you will not be able to access your form W-2/1042-S/1095-C through the System HR website (My UI Info). However, you will be able to retrieve your form W-2 or 1042-S, and/or 1095-C from our vendor's website mid-January. 

To access the Yearli Portal as a separated former employee, you must enter the email address you provided during enrollment or from a previous login.

If you have not updated your email address, the Yearli Portal will require you to use the address from the previous ePrintview system. If you cannot access the email address, contact UPB from the University Payroll & Benefits Service Portal to set up an email address you have access to.

The following instructions will guide you through the retrieval process. If it is your first time logging directly into the Yearli Portal, expand the instructions below.

First-time direct login to Yearli

  1. Log in to the Yearli Portal.
    1. Type in your email address.
    2. Enter the last four digits of your Social Security Number (SSN).
    3. Select your preferred language.
    4. Click the Next button.
  2. Verify your account
    1. Enter the verification code sent to your email address
    2. Click the Verify code button. 
  3. Create your password.
    1. Enter and confirm a new password according to the password requirements (at least 10 characters, one lower case, one upper case, and one digit).
    2. Click the Create button.
  4. Set up multi-factor authentication.
    1. Select the form of multi-factor authentication you wish to use and click the Continue button:
      • Phone
        1. Enter your phone number.
        2. Click the Text Code button to receive an SMS text of your code to your phone.
          OR
          Click the Call Phone button to receive a phone call with your code.
      • Authenticator App
        1. Scan the QR code from your authenticator app.
        2. Click the Continue button.
    2. Enter the verification code sent to your phone or authenticator app in the Enter code field.
    3. Click the Verify Code button.
  5. Select the tax form you wish to access. 
    NOTE: If you have not enrolled in paperless delivery for the form you selected, you will receive a Form Not Available message.
  6. Your Form W-2/1042-S/1095-C will display in your browser. Save or print.

    Direct login to Yearli

    1. Log in to the Yearli Portal.
      1. Type in your email address.
      2. Enter the last four digits of your Social Security Number (SSN).
      3. Select your preferred language.
      4. Click the Next button.
    2. Enter your password and click the Log In button.
    3. Authenticate your account.
      1. Follow the instructions below for your type of authentication:
        • Phone
          1. Click the Text Code button to receive an SMS text of your code to your phone.
            OR
            Click the Call Phone button to receive a phone call with your code.
          2. Enter the verification code in the Enter code field.
          3. Click the Verify Code button.
        • Authenticator App
          1. Enter the verification code sent to your authenticator app in the Enter your code field.
          2. Click the Continue button.
    4. Select the tax form you wish to access. 
      NOTE: If you have not enrolled in paperless delivery for the form you selected, you will receive a Form Not Available message.
    5. Your Form W-2/1042-S/1095-C will display in your browser. Save or print.
