ATLAS FormBuilder - Adding and Editing Questions

This document explains how to create new questions within the contents of a FormBuilder form, and how to edit existing questions.

Questions are highly-customizable data entry fields that can be used to populate the contents of a form.

Like all data inside a form's contents, questions can only be created within sections. For information on adding new sections, visit ATLAS FormBuilder - Adding and Editing Form Sections.

To create a new question or edit an existing one...


Step 1 - Navigate to your form's dashboard and click Manage in the header of the "Form Contents" panel.

Step 2 - If creating a new question, first select a phase that contains the section to which you'd like to add a question. Then, locate and select the section in the Form Preview panel, and click the Add Question button that appears in the Form Contents panel.

If editing an existing question, first select the phase containing the question you wish to edit. Then, locate and select the question in the Form Preview panel, and click the Edit button that appears in the Form Contents panel.

Step 3 - A settings panel for the question will open beneath the Form Contents header, containing several fields for editing the question's attributes and behavior.

The available fields include:

  • Question Text: The text that accompanies the question in the contents of the form. Can serve as the questions's title or description.

  • Field Name: The internal name of the question. This field's value is modified automatically as the Question Text field is filled out, but it can be customized as well.

  • Additional Instructions Text Format: The text format that the Additional Instructions field will use. Allows for choice between Markdown and PlainText formats.

  • Additional Instructions: Optional text that will display below the question in the form contents. Often used for instructional information that is too long for the Question Text field.

  • Is Required: Determines if the question is required or not. Required questions will not allow a form response to be progressed (submitted or advanced to the next page) until they are provided with an answer.

  • Is Active: Determines if the question is active or inactive. Inactive questions can still be modified at the admin level, but will not be present in the form's contents.

  • Type: Allows for selection from a variety of "question types," which determine the primary functionality and purpose of the question. Depending on the selected question type, additional fields for modifying the question's behavior will display below this field.

  • This question is triggered by another question's value: When this option is enabled, it allows for the setup of a trigger that can modify the question's value, required state, or visibility —  based on the answer to another question.

Step 4 - Once you're finished making changes, click the Save Changes button at the bottom of the settings panel.

Your new or newly-edited question will be ready to use, and its changes will be immediately applied to the form's contents!
