Policy
All University DEA license holders must use their university DEA number only for university business and in accordance with all university policies. A university DEA number can be identified by the first letter being an F and the use of the license is limited to official university duties.
Guidelines
- University DEA numbers are fee exempt for doctors actively working at the University.
- Only doctors actively providing university service through research or clinical work may hold a fee-exempt university DEA license.
- Upon retirement or separate from the university, the DEA registration must be canceled.
- All DEA use must comply with all federal and state laws and university policies.
Procedure (if applicable)
- After obtaining a state-controlled substance license for the appropriate practice address, apply for a DEA license through https://www.deadiversion.usdoj.gov/.
- When completing the DEA application, check the box for “Certification for Fee Exemption”.
- For everyone with a VTH clinical appointment (>0%), the certifying official will be the current Hospital Director.
- For those requiring a DEA number for research but who do not have a VTH clinical appointment, the certifying official will be the current Department Head.
- For the Hospital Director and the Department Head, the certifying official will be the current Dean of the College of Veterinary Medicine.
- Once the license is received, a copy must be provided to the current Pharmacy Coordinator.
- When separating from the University, the DEA license holder is responsible for notifying the DEA that they are no longer employed by the University and that their license should be canceled. This includes retirees that no longer have a need to store controlled substances at the address listed on the DEA license. During the off-boarding process, the license holder will be provided with information on how to cancel their university license.

DEA: Drug Enforcement Administration
University DEA Number: A DEA number for an individual that is associated with a university address and was marked fee-exempt.
Veterinary Teaching Hospital (VTH): The collective clinical services of the Large Animal Clinic, Midwest Equine, the Small Animal Clinic, and the Veterinary Medicine South Clinic.
