Policy

To ensure the safety of drugs and products that may have a question in safety or usability. This applies to items that have potential health hazards, are mislabeled, potentially contaminated, or poor quality. The Medication Dispensary will investigate the products with the affected lot/NDC in the dispensary setting and products kept within the hospital. If affected products are found, products will be replaced free of charge to departments if unaffected new products are available. 

Guidelines

The Medication Dispensary keeps records of Recall Notifications and documentation of affected drugs, if applicable, and procedure of how it was handled.

Procedure (if applicable)

  1. Once reports or notifications are known, dispensary staff will investigate on where all the drugs/products are located. Staff will verify the NDC or lot numbers to confirm if the hospital has any affected packages. 
  2. If there are no recalled products within the VTH or VMSC setting, staff will record “No Affected lots in VTH/VMSC” and date and initial the notification. Dispensary will keep a binder of Recalls for record-keeping. 
  3. If there are confirmed recalled products found, staff will take the necessary steps based on the notification instructions. This may include properly disposing of the drug or sending back the recalled products to the Manufacturer or Vendor purchased from. 
  4. To notify VTH / VMSC staff, dispensary will send an email to all staff necessary to confirm no products are left unchecked and the necessary steps to take if some are found.
  5. Run a report to verify which clients received recalled products and contact them to offer them a solution (such as replacement), or necessary precautions to take or how to properly dispose of medication and who to contact for additional questions.

Veterinary Teaching Hospital (VTH): The collective clinical services of the Large Animal Clinic, Midwest Equine, the Small Animal Clinic, and the Veterinary Medicine South Clinic.
