Code requests should include the following:

1. The Description you want to appear. 
   • There is a 55-character limit for the description
2. The Standard charge on professional fees/services
   • HIS will set the price in SANDI exactly as you specify.
3. The cost to us on inventory/supply items. 
   • HIS will calculate the markup and set the appropriate price in SANDI. The policy for pricing is here: Set Pricing for Professional Services, Drugs and Medical Supplies 
4. Add Shipping:
   • Note if you want shipping to be added automatically with the charge.
   • Below is are the codes and prices as of 5/1/24, always check current pricing in SANDI. GS203 is the most commonly used shipping cost.

6. Include any other information you think is pertinent for set up.
