This page contains information about online survey tools available to the campus community.
The University of Illinois has negotiated agreements to provide several survey tools to members of the University of Illinois at Urbana-Champaign. Although these tools allow online surveys to be conducted with relative ease, there are considerations that must be taken into account before launching a survey. This web page will help guide you through some of the procedures and considerations for using these survey tools.

Cost varies based on subscription (see Qualtrics product page for details). Qualtrics can be purchased through Banner/CFOP account.

In addition, members of the UIUC community can receive 50% off of an annual subscription. To qualify for this discount, Survey Gizmo asks that the text reading "Powered By Survey Gizmo" be retained at the bottom of surveys.

Customers should contact Qualtrics directly for support.
Technical support for Survey Monkey, Survey Gizmo, and ZipSurvey will be provided by the survey companies themselves. Questions, problems or concerns should be directed to the individual service that is being used, and not to Technology Services or the University of Illinois.
Click Thru Agreements
The terms of the contracts negotiated by the University of Illinois supersede any conflicting language found within the survey companies' individual click thru agreements. Most of the conflicting language concerns procurement and purchasing issues, and therefore, the average user of these services will not be affected by language conflicts.
Institutional Research Board Considerations
Before using any of these survey tools, it is important to consider if your project falls under the purview of the University of Illinois Institutional Review Board. According to the IRB's web site, "(a)ll research involving human subjects that is conducted at or sponsored by the University of Illinois at Urbana-Champaign (UIUC), whether funded or unfunded and whether conducted by UIUC faculty or others, must comply with applicable policies for the protection of human subjects."

Web Privacy Notification
Any survey conducted online also has to be aware of considerations that could be raised by the University of Illinois Web Privacy Notice. When it comes to online surveys, the Web Privacy Notice specifically states:
In the course of using University Web sites, a user may choose to provide information to help the University of Illinois serve that user's needs or conduct research. At any time there are numerous online surveys being conducted on University Web sites. Person(s) responsible for conducting online surveys that collect personally-identifiable information should clearly state at the survey site the extent to which any information provided will be shared. Aggregate data from surveys may be shared with external third parties in ways that do not compromise privacy.Not only should the researcher be aware of this provision, but it is recommended to link to the Web Privacy Notice so that survey respondents can understand their online privacy rights that are granted to them by the University.
