Teams, Do I need to complete the E911 acknowledgement form?

All Teams Enterprise Voice enabled customers must complete the E911 Acknowledgment form designed by University Counsel in accordance with FCC regulations.

The form must be signed to activate the customer's number for Teams. The form is available at: http://uc.illinois.edu/e911acknowledge.

Customers can confirm whether or not they are Enterprise Voice enabled at: http://uc.illinois.edu/myuc. If "Enterprise Voice Status" shows "Enabled" the customer is required to complete the E911 Acknowledgment form. The date and time of acknowledgment show for "E911 Disclaimer Acknowledged" if it has already been done.
