- Complete E-9-1-1 Acknowledgement Form*
- Request your Exchange email account
* The Federal Communications Commission, through the Code of Federal Regulations (CFR), requires the University to obtain and keep a record of affirmative acknowledgement by every Skype for Business customer (also called Unified Communications or UC users), both new and existing, of having received and understood the advisory statement provided on the E-9-1-1 Acknowledgement page. It is required that Skype for Business users read the advisory statement about E-9-1-1 and Skype for Business (also called Unified Communications or UC) and acknowledge that they have read the statement. Technology Services can not provide Skype for Business phone service for a customer until the customer completes the E-9-1-1 Acknowledgement Form.
