ATLAS FormBuilder - Make a Simple Form PT 1

This is part 1 of a 4 part tutorial that will take you step by step (click by click) through the process of building a basic form using ATLAS FormBuilder. This is not necessarily how your official forms will function; it is instead a contrived example to display the basic steps involved in building a form.

To build your own test form alongside these tutorials, you'll need access to the ATLAS FormBuilder testing space.

If you do not have access to the testing server please email atlas-formbuilder@illinois.edu


Step 1 - Click 'Forms'

Click Forms


Step 2 - Click  '+ Add New Form'

Click "Add New Form"


Step 3 - Type in "Your Favorite Day" in the 'Form Name' box provided, and then hit 'Save'

Type in "Your Favorite Day" in the box provided and then hit "Save"


Step 4 - In the Data Collection and Form Lifecycle box, click 'Phases'

In the Data Collection and Form Lifecycle box Click "Phases"


Step 5 - Click '+Add Phase'

Click "Add Phase" 


Step 6 - Type "Your Favorite Day" in the 'Title' box and then click '+ Add Permission'

Type "Your Favorite Day" in the Title* box and then click "+ Add Permission"

Step 7 - Under USERS check 'Anonymous Users'. Under PERMISSION LEVEL check 'View', 'Edit', and 'Requires Submit'. Click red 'Save' button

Under USERS check "Anonymous Users". Under PERMISSION LEVEL check "View", "Edit", and "Requires Submit". Click red "Save" button


Step 8 - Click the blue 'Save' button

Click blue "Save" button.
