ATLAS FormBuilder - Accessing Form Responses
All responses to a FormBuilder form during a given form period are stored in a chronological and sortable list in the form's back-end.
From this response list, "details" pages of individual form responses can be accessed. A response's details page displays its history, allows for viewing and editing of its current phase and its answers to form questions, and provides easy access to general information like its unique survey site URL and its last modification date.
To access a form's response list and response details pages...
Step 1 - Navigate to your form's dashboard, and select one of the following options to open the Form Responses page:
- The Form Response List button in the "Quick Links" section
- The Manage link in the header of the "Response Data" panel
- The Number of Responses link inside the "Response Data" panel

Step 2 - On the Form Responses page, you will be presented with a chronological list of the form responses submitted for the current period, as well as numerous settings for controlling the filtering of the response list. These include:
- A drop-down menu for selecting the period whose response list should be displayed.
- A field that accepts the input of a NetID or group name, filtering the list down to only include responses submitted by the entered user or group.
- A drop-down menu for selecting the maximum number of responses that can be displayed per page of the list, between 10, 25, 50, and 100.
- A search field that can be used to filter the response list based on the text present in any column.

Once you've located a response you wish to view, click its Details button.

Step 3 - You will be brought to the "details" page of your selected form response, which presents a wealth of information about the response and controls for modifying its key attributes. At the top of the page is the response's unique alphabetic identifier.

The rest of the page's information is divided into distinct sections. These sections are as follows:
- Status - Provides general information about the form response, and provides several controls for actions such as changing the response's period, modifying its registration (when applicable), and deleting it entirely.
- The section's fields are as follows:
- Submitted By: The display name and email address of the response submitter.
- Submission Date: The date and time of the response's submission.
- Last Modified: The date and time of the response's last modification, as well as the display name and email of the last modifying user.
- Survey Form: A clickable link to the response's survey site page.
- Period: The form period associated with the response.
- The section always provides the following controls:
- Change Period: Opens a window that allows for the response to be associated with a different form period.
- Delete Form Response: Opens a window that allows for the form response to be deleted after a confirmation step. For further information, visit ATLAS FormBuilder - How to Delete a Form Response.
- Test Access to Form Response: Opens a window that can be used to test a specific user's access to the response in its current state on the survey site.
- The following controls are only available in event registration forms:
- Add to Waitlist: Adds the response to the form's waitlist. Only available for forms that have enabled a waitlist, and for form responses that are not registered.
- Register Response: Force-registers the response. Only available for form responses that are not registered.

- The section's fields are as follows:

- Current Phase - Displays a diagram of the form's workflow, with the current phase of the response highlighted in red. All phases and phase-changing routing triggers used by the response will be colored black, while unused phases and routing triggers will be colored gray. A button below the diagram can be clicked to display the response's workflow in text format.
The section's header contains a Change Phase button, which opens a window that can be used to manually set the response to a different phase.

- Export - Provides clickable PDF download links for the form's data dump and any custom PDF reports created.

- History - Displays a chronological list of all edits made to the form response, including every time the response is saved, every time the phase is changed, and every time an email is sent.
Each entry in the list possesses its own Details button, which displays a window containing further details about the entry, such as which specific questions or pieces of data were modified, or which trigger was responsible for changing a phase.

- Answers - Presents each of the form's questions and their current answers provided by the response. The answers to File Upload questions are presented as clickable download links.
