IMS Research Account Creation Request Form (Used to Purchase inventory via the IMS or charge a service via SANDI)

Please follow the link below to complete a Research Account Creation request to purchase of Inventory and Dispensary items via the Inventory Management System.(IMS).

Research Account Creation Form 

All VTH Storeroom and Pharmacy sales are now requested and fulfilled via the Inventory Management System (IMS). If you need a user account in the IMS please email his@vetmed.illinois.edu to request an account. 

IMS FOAPAL Add/Changes: If you are ONLY in need of a FOP addition, no DBF, in the Inventory Management System (IMS) you can make that request here: IMS CFOP Addition Request
IMS SANDI Service Account: This is ONLY if the research account will need to be charged for a SERVICE, inventory, and supplies should all go through the IMS.
Note: It is very rare, but there are situations where a Research Account still needs to be created in SANDI. This will only happen when the research account will need to be charged for a service versus an inventory procedure code. An example is the need to charge imaging studies to the research account.

There is a line at the bottom of the VTH Research Account Creation form that asks the researcher if they will utilize non-inventory/supplies services in the hospital such as imaging? If they answer yes, please send a copy of the form to his@vetmed.illinos.edu and HIS will create the SANDI account.

Form Verbiage: Will you require a SANDI account to allow billing of non-inventory/supplies services in the hospital, such as imaging studies? (This is not common/typical)
