Policy
This policy is to outline the process by which VTH drug formulary additions and deletions occur, and this policy covers all drug items stocked by the VTH dispensary and VMSC.The VTH drug formulary will reflect clinician prescribing preferences.
Guidelines
- When a new medication is requested, the pharmacy coordinator will determine client cost and have it approved by faculty on the requesting service.
- If client cost is approved by faculty, then the medication will be stocked. If the medication expires, then it will be reevaluated as described in the expired medications policy.
- Compounded medication requests will be evaluated based on feasibility of compounding as well as stability, and efficacy data.
Procedure (if applicable)
- When a new medication is desired, the clinician should submit a formulary request form to the dispensary. These forms are available on the intranet and in the file bin outside the dispensary.
- The pharmacy coordinator will evaluate the availability and cost of the medication and see if there is anything similar already stocked.
- The pharmacy coordinator will follow up with the faculty on the requesting service with the client cost and any additional information to reach a final decision.
- If the medication is available and the client cost is considered acceptable, the medication will be ordered in a small quantity. PAR levels will be adjusted based on use patterns.
- If the pharmacy coordinator and requesting service cannot come to an agreement on whether to stock a medication, the request will be sent to the Hospital Board with all supporting information for the board to make a decision

Veterinary Teaching Hospital (VTH): The collective clinical services of the Large Animal Clinic, Midwest Equine, the Small Animal Clinic, and the Veterinary Medicine South Clinic.
