Policy
The intent of this policy is to describe the process for authorized personnel obtaining and using Small Animal Junior Surgery drugs. Junior Surgery Kit must be kept in Cubex when it is not in use. The surgery kit must be obtained from Cubex, and appropriately logged when being used Surgery kit is to be returned when procedures are done.
Guidelines
- Drug vials will be charged in the HIS (Health Information System) to the appropriate account (Junior Surgery or unless otherwise directed)
- Yellow Log sheets will be attached to each drug vial and will require documentation with any withdraw
- When either drug is running low, it is the responsibility of the Department personnel to notify Pharmacy to order another vial.
- When vial(s) need to be restocked, Pharmacy staff will stock the Cubex.
- Any quantity or log withdraw discrepancies will require investigation on finding the error.
Procedure (if applicable)
Log into Cubex and select the following:
- Patient: Select JR Surgery Kit
- Select Item/Drug: SA JUNIOR SURGERY *ANESTHESIA* KIT for Anesthesia Junior Surgery staff or SAS JUNIOR SURGERY KIT for Junior Surgery staff.
- Document any withdraws on the yellow log sheet.
- Return Kit to Cubex when the procedure is done. To return, log in and select the same patient (Jr Surgery Kit) and select “Return Item(s)” at bottom of the screen. The Jr Sx Kit will automatically show up as a Returnable Item. Select the Kit, and choose “Return Now”.
*If returned incorrectly, it will show a quantity of 0 and will require Pharmacy to fix this. - When a drug vial is empty, please bring to Pharmacy or leave in Cubex. Notify Pharmacy if another vial needs to be ordered and prepared.

Veterinary Teaching Hospital (VTH): The collective clinical services of the Large Animal Clinic, Midwest Equine, the Small Animal Clinic, and the Veterinary Medicine South Clinic.
