Adding a Report Image

1. Select Exams & Forms from the Patient Dashboard by selecting the arrow.

2. Select Add (Blue button in upper left corner of the Exams/Forms box.)

Visit Number

• If there is an open Visit for the patient, the Visit Number (and detail) will be displayed - see example below.
• If there is no open Visit, you should select one from the drop-down. ALWAYS associate the proper Visit in the drop-down

Note: If a Visit is not associated, you will not be able to Send the report to the owner, RDVM, etc.  Email medrec@vetmed.illinois.edu for assistance with exams lacking the Visit number.

4. Select Exam Type and select Imported Videos/Pictures and Submit (AGAIN, it is VERY important that you associate a Visit or you will not be able to Send the report.) 

5. The report will show in the list of forms.  Select the Edit icon.

6. The upload screen will appear. 

1. Select Upload New File
2. Enter a descriptive Label. (This will be viewable when editing or viewing the uploaded image(s).)
3. Drag and drop or browse for the image(s) you want to upload
4. Verify on Upload - Use this if you are ready to verify the entire exam. (This will fully verify the exam, and no further changes will be allowed.)
5. Select Upload File to complete the upload.

Notes:

• You can verify the Exam at any time by choosing the Verify Exam button in edit mode.
• There is a 100MB limit for uploaded files. If your file exceeds this limit, you will be notified within the upload screen.
• Uploading PDFs is allowed, but those over 10MB may take a few seconds longer to upload/generate, and screen instructions will appear.

7. If the exam was NOT verified, you have options to edit and/or delete.

8. Close the Upload window, and you will see your upload listing. You may choose to edit if in draft.

9. You may send the image just as you do with any other report.
