Policy
Policies define and outline the procedures that are integral to VTH operation. Policy creation is based on need and may be initiated by VTH personnel. Policy adoption is subject to administrative review and authorization. The reviewing authority is based on the nature and content of the policy. Policies will include a summary of the policy, guidelines for policy application, and procedures for use. Approved policies will be archived on the VTH Administration drive and will be posted on the VTH intranet. Policies will be reviewed a minimum of once every three years, or more frequently if needed.
Guidelines
Policies governing VTH operation may be proposed by the Administrative Team, Hospital Director, and/or VTH and VCM Committees at any time. The proposed policy should be forwarded to the Hospital Director. The Hospital Director is responsible for review and approval of policies pertaining to VTH processes and operational needs. Policies pertaining to VTH strategic planning and development will be presented to the VTH Board by the Hospital Director for review and approval. Newly approved policies will be emailed to VTH personnel when posted to the VTH intranet. Policies will undergo a regular and reoccurring review. Policy review will be performed by the appropriate personnel having direct knowledge of the policy subject matter. Once review and/or revisions are complete, the policy will be forwarded to the Hospital Director for approval or to be forwarded to the VTH Board as described above. Reviewed policies will be emailed to VTH personnel only if significant revisions result in operational changes.
Procedure (if applicable)
A policy template will be maintained by the VTH Administrative Office. Persons wishing to develop a VTH policy will contact the office to obtain an electronic copy. Once completed, the proposed policy is sent to the Hospital Director for consideration.
Policy review will be initiated by the VTH Administrative Office, with a review scheduled as needed but not to exceed every three years.
