Policy
This document outlines the process of transferring C2 inventory ordered under the VTH DEA number to the VMSC DEA number. This applies to all C2s kept as stock at VMSC. All C2s must be transferred using a DEA Form 222.
Guidelines
DEA Form 222s are available for Dr. Whittington (VSMC) and Dr. Sander (WMC). When C2s are transferred for Ward Stock in either location, a DEA 222 form must be completed, and all required copies made and filed. The supplier copy must also be scanned and emailed to the DEA.
Procedure (if applicable)
- Obtain the appropriate DEA Form 222 (Dr. Whittington for VMSC and Dr. Sander for WMC).
- Complete the purchaser’s portion of the form (part 1 and part 2)
- Obtain a signature from the DEA registrant or a designated power of attorney (POA)
- Make a copy of the form
- Complete the supplier section (part 4)
- Scan the form and send to dea.orderforms@usdoj.gov
- Complete part 5 on the copy made in step 4
- File the copy in the VMSC 222 – Purchaser folder
- File the original form in the VMSC 222 – Supplier folder
- If an error is made on a 222 form it must be voided by writing void across the face. File the voided form in the Voided VMSC 222 folder

Veterinary Teaching Hospital (VTH): The collective clinical services of the Large Animal Clinic, Midwest Equine, the Small Animal Clinic, and the Veterinary Medicine South Clinic.
