Policy
To describe the process of accessing and administering a hydromorphone single use vial for treatment of the same patient throughout the day. The Hydromorphone 2mg/ml vial must be properly labeled with patient, and “Date Opened”. It must be stored promptly after usage and must be expired at the end of the day.
Guidelines
- Each vial is for that patient specifically and cannot be used for another patient.
- The vial has to be expired the end of the day, no exceptions.
- Dispensary will conduct routine audits and will check Cubex transactions, purple sheet documentation, and properly drug waste. If the procedures and guidelines are not properly followed, Dispensary will take away this storage option.
Procedure (if applicable)
- When the single-use vial (code Hydromorphone 2mg/ml SDV 1ml) has been taken out for that patient, staff must attach a patient label to the vial AND “Date Opened” sticker.
- Once dose has been taken out and needs to be kept in Cubex for the next use, sign in Cubex, follow the same procedures (select patient, doctor), then select the Hydromorphone Patient Med code to open the bin to place open vial in. When it is needed again, take this out under that same code.
*Since this bin is to securely store an open item, it will not have a countback screen, or an accurate “quantity on hand” number on the screen* - Once patient has been discharged, or treatment of Hydromorphone has been discontinued, or if it is the end of the day –this must be wasted in Cubex. This is done by wasting the remaining ml under that patient in Cubex. (Draw up this quantity in a syringe to have an accurate amount) Please enter this quantity wasted in the comments section
