University Bursar - How do I set up an authorized proxy to view my Form 1098-T?

Proxy Access allows a student to authorize other people to view and access the student's 1098-T tax information. A student can designate a parent or guardian as a proxy. Please note that proxy access does not provide access to the student account.

To set up an authorized proxy:

1  Select your university from Self-Service

2 Enter your Net ID and Password and click "Log in"

3 Verify your identity

4 In the “Student Self Service” card, choose Home Page

5 Click on the 3 lines on the top left and choose “Banner Self Service” from the menu

6 Click "Proxy Access Menu" and then select "Proxy Access"

7 Click "Add New" and enter proxy information

8 Notify your designated proxy once they are set-up because action will be required

9 The Authorized User (Proxy) will receive several email notifications with temporary password and link. The Authorized proxy must confirm the email and re-set the password.

Please Note: Proxy access expires after 18 months, unless a shorter alternative date is selected by the student.
